Forms and documentation

Sick Notes and Medical Forms in Mississauga

A clinician must assess whether a note or form can be completed. Many documentation services are not covered by OHIP; fees are confirmed before the service.

  • Work and school notes
  • Insurance forms
  • Driver forms
  • Transparent fees

What to bring

Bring the complete form, identification, your health card, and any records or employer instructions relevant to the request.

  • Do not sign sections meant for the clinician
  • Allow enough time before your deadline
  • Complex forms may require records or a separate appointment

Fees and completion

The clinic posts common uninsured fees on the Walk-In page. Final cost and turnaround depend on the form and assessment required.

Can I get a sick note at a walk-in visit?
A physician can assess your condition and determine whether documentation is appropriate.
Are medical forms covered by OHIP?
Most forms and notes are uninsured services. Review the posted fee schedule and confirm the price at check-in.
Will my form be completed immediately?
Simple documents may be completed during the visit. Complex forms can require additional records, time or a follow-up appointment.
